Designer Laser Vaginoplasty® (DLV®) is the aesthetic surgical enhancement of the vulvar structures, labia majora, mons pubis, perineum, introitus and hymen (see figure). DLV® has provided creative treatment solutions for patients from over 50 states and 30 countries. This has provided us with a vast experience and creative expertise to aesthetically design procedures in line with the specifications and desires of the patient.

Although our experience and techniques provide us the ability to design most anything that one can desire, our most common DLV® procedures are as follows:

Laser Reduction Labioplasty can sculpture the elongated or unequal labial minora (small inner lips) according to ones specification. Most women tell us that they do not want the small inner lips to project beyond the large outer lips. Many women bring us Playboy and say that they want to look like this. With laser reduction labioplasty, we can accomplish the desires of the woman. Our labioplasty techniques can also reconstruct conditions that are due to the aging process, childbirth trauma, or injury.

Laser Reduction Labioplasty with Reduction of Excess Prepuce (excess skin along the sides of the clitoris) can provide the true playboy aesthetic look. Women tell us that they want the skin to drape neatly over the clitoris. The reduction of the excess prepuce provides the desired aesthetic enhancement of the clitoral skin. The majority of women having laser reduction labioplasty need the reduction of excess prepuce for a complete aesthetic result.

Laser Perineoplasty can rejuvenate the relaxed or aging perineum. It can also enhance the sagging labia majora (large outer lips) and labia minora. Overall, the procedure can provide a youthful and aesthetically appealing vulva.

Augmentation Labioplasty can provide aesthetically enhanced and youthful labia majora by autologous fat transplant (removal of the patient's fat via liposculpturing and transplanting it into the labia majora).

Vulvar Lipoplasty can remove unwanted fat of the mons pubis and upper parts of the labia majora. Liposculpturing can alleviate the unsightly fatty bulges of this area and produce an aesthetically pleasing contour.

Combination of DLV® with LVR LVR® and DLV® can be performed in combination. They can also be performed with most cosmetic surgical procedures; the most popular of these are liposculpturing, breast implants, breast reduction, tummy tuck, nose surgery, and eyelid surgery.

Vaginal Relaxation and Functional GYN Problems

Thirty million American women suffer from symptoms of vaginal relaxation and stress urinary incontinence. Many women have difficulty controlling their urine in certain situations or notice changes in their bowel habits. These two symptoms may be related to a common set of problems that may occur as a result of childbirth, aging or a combination of both. Grouped together these problems are referred to as pelvic relaxation.

Many women suffer unnecessarily from conditions involving pelvic relaxation. Appropriate diagnosis and treatment will often restore patients to a life free of the aggravations and discomforts associated with pelvic relaxation.

The goal is to acquaint you with the various forms of pelvic relaxation as well as their causes, symptoms, and treatment. The pelvic organs include the vagina, uterus, bladder, and rectum. These organs are held in position by three types of supports: 1) muscles, 2) sheets of tissue called fascia 3) and ligaments. When these supports become damaged for various reasons, one or more of the pelvic organs may sag and, occasionally, even protrude outside the vagina. These are called pelvic support defects.

During childbirth, as the baby passes through the birth canal, the muscles, fascia, and ligaments separate and may be weakened. This weakening gradually worsens and, in later years, may cause the pelvic organs to drop from their normal positions.

Occasionally, this weakening of the muscles and tissue may occur in women who have never had children. In these women, the cause may be:

Inherited weakness of the supporting tissues.
Unusual strain placed on the supporting tissues by a chronic cough.
Unusual increases in abdominal pressure.
Obesity.

The general symptoms associated with pelvic relaxation depend on which organs are affected. Often there is a feeling of heaviness or fullness. Small or moderate amounts of urine may be lost with normal physical activities such as laughing, coughing, walking, or running. In more advanced and rare cases a mass may actually protrude from the vaginal opening. Based on the organ or organs involved, pelvic support defects can be defined more specifically as:

Cystocele.
Urethrocele (most of the time the cystocele and urethrocele occur in combination - cystourethrocele).
Rectocele.
Enterocele.
Uterine prolapse.

A cystocele occurs when the bladder falls or descends from its normal position. The most common symptom associated with cystocele is difficulty in completely emptying the bladder. This can be associated with bladder infections. Large cystoceles can cause the bladder to overfill and allow small amounts of urine to leak. Leakage is most common during activity such as walking or bouts of coughing.

A urethrocele usually occurs in conjunction with a cystocele. Both of these conditions result in, among other things, involuntary loss of urine, particularly when there is increased pressure in the abdomen, caused by walking, jumping, coughing, sneezing, laughing, or sudden movements. Rectoceles happen when the rectum bulges into or out of the vagina.

Rectoceles usually occur as a result of injuries sustained during childbirth. With a weakened or bulging rectum, bowel movements become more difficult.

An enterocele is the bulging of small intestines into the back wall of the vagina.

Uterine prolapse occurs when the uterus falls or is displaced from its normal position. There are varying degrees of severity depending on the descent. This produces a general felling of heaviness and fullness, or a sense that the uterus is falling out.

The diagnosis of these problems includes a through history and physical examination. Other test depending on the circumstances include a "Q-tip" test, urodynamic studies (a painless fifteen to twenty minute computerized bladder and urethra functional studies), urethrocystoscopy (instrument used to evaluate the inside of the bladder and urethra), X-rays of the urinary system.

For all practical purposes, definitive treatment is surgical correction of the specific defects.

Laser Vaginal Rejuvenation® (LVR®) is a one-hour outpatient surgical procedure designed to enhance sexual gratification. According to Master and Johnson, sexual gratification is directly related to the amount of frictional forces generated. We can accomplish this with LVR®. LVR® is the modification of a gynecological surgical procedure used for the treatment of stress urinary incontinence. Our laser techniques result in gentle precision procedures with controlled accuracy that results in relatively bloodless surgery. Our pioneered techniques result in rapid healing and resumption of daily activities in a short period of time.

Laser Vaginal Rejuvenation® (LVR®) will effectively enhance vaginal muscle tone, strength, and control. Women are multiorgasmic. The female sexual response cycle is unimpeded by a required refractory period that prevents successive orgasms in the male.

Principal understanding of the physiology of the human sexual response emanates from observational research studies by William H. Masters, M.D. and Virginia E. Johnson. The researchers tested 382 women and 312 men in more than 10,000 episodes of sexual activity. Laser Vaginal Rejuvenation® for the enhancement of sexual gratification is the modification of a standard existing gynecologic vaginal surgery used throughout the world for the treatment of vaginal relaxation and its associated symptoms, primarily that of stress urinary incontinence (involuntary loss of urine with stress such as exercise, etc.)

Vaginal relaxation is the loss of the optimum structural architect of the vagina. In the vaginal relaxation process, the vaginal muscles become relaxed with poor tone, strength, control and support. This can be done by removing a small amount of fat via liposculpturing from a particular area such as the abdomen, hips, or thighs. The fat is then processed and a small amount is injected into the fat of the labia majora. This provides a beautiful and natural enhancement of the labia majora.

DLV® -Vulvar Liposculpturing can completely remove the unsightly fatty bulges of the mons pubis (pubic area) and labia majora. The fatty accumulations in these structures produce clothing, undergarment, and bathing suit irregularities and bulges. In some cases it can prevent women from wearing clothing of their choice. DLV® -Vulvar liposculpturing will aesthetically enhance, reduce and define the structures.

Vaginoplasty is a standard gynecologic surgical procedure. It tightens vaginal muscles and surrounding soft tissues, by reducing excess vaginal mucosa (vaginal lining). The result is an immediate decrease in the size of vaginal muscles, resulting in more friction during sexual experiences.

After surgery, the patient is usually able to walk comfortably within a few days and may return to sexual activities within 4-6 weeks.

Surgical risks are few, but may include among others: infection, bleeding, and scarring.

Vaginoplasty

In general it represents modification of female vagina. Most frequent procedure is the narrowing of vaginal opening, eventually specific narrowing of the vagina, which can lead to its partial firmness.
The aim of the surgical procedure is to return, eventually enable better sexual life to the patient.
This surgery is usually performed in women of middle age that gave birth already and when it came to loosening of vagina and vaginal opening caused by deliveries and increasing age.
The procedure is performed under general anesthesia, it lasts usually 45 minutes. It is performed with following technique: In the area of vaginal opening, on its back side, an excision of a tissue in various extents (according to the wish of the patient, anatomical finding and so on) is made. Then the wound is sutured. It is better to make the extent of the excision bigger and therefore narrow the opening more because consequently during the life it is usual that the vaginal opening widens again slightly.
If the incision is expanded even to the back vaginal wall and into different distances, we can contract the whole vagina like this. This surgery is also performed under general anesthesia, it lasts around 1, 5 hours.
The surgery does not involve only the mucosa, but it intervenes even into the muscle layer of vaginal wall. At first the vaginal mucosa is excided in needed (chosen) extent, next the muscle layer of vagina is firmed and shortened and the last phase of the surgery is to suture the vaginal mucosa. The same is valid about the extent of the excision as it is mentioned in correction of vaginal opening. During this surgery a big prudence and experience of the surgeon is necessary because of the near vicinity of vagina and terminal part of colon to prevent the creation of so-called rectovaginal pouch.
Both methods of the surgeries use absorbable suturing material, we do not recommend sexual intercourse, sometimes even for 6 weeks.

Labiaplasty

Labia minora

This term means surgical modification of inner and outer lips in women.
The labia minora in women often protrude between the labia majora and many times reach unbelievable size. The aim of the surgery is to reduce such protruding inner lips, sometimes even almost completely remove. According to their function we do not recommend their total removal.
The surgery is not usually difficult, it can be performed even under local anesthesia, analgosedation, or eventually under general anesthesia. We use absorbable material and we do not recommend a sexual intercourse for about one month.

Labia majora

Atrophic labia majora, when the fat tissue and elasticity reduce in time and therefore the labia loose their fullness.
There are several ways of solution:

Filling of labia with own fat. The procedure is usually often repeated because own fat can be very soon absorbed according to the frequency of sexual activities.
Another method is the skin reduction of labia majora, when the incision is usually made in the inner side to labia minora.
The combination of both methods.

It is possible to perform all the surgeries even under local anesthesia, although it is better under analgosedation or general anesthesia.
Hypertrophic (excessively large) labia majora
Most often in young women, there are also several ways of solution:

Liposuction,
Surgical reduction, when the incision is led the same way as in skin reduction of atrophic labia,
The combination of both methods.

Hymenoplasty – Hymen reconstruction surgery

A lot could be written about the motives for which women decide to undertake this surgery.
There are several methods, although they never mean the real hymen reconstruction. It is always just a surgery with the only one aim to have bleeding during the sexual intercourse so that the man has the feeling that he was the first man for his partner.
Most often a thin and short small flap of mucosa is separated from the vaginal wall (approximately 1/3 of vaginal width), it is then sewed very gently to the opposite vaginal wall. Like this the vagina is not blocked too much to let the menstruation blood flow away, although at the same time it comes to bleeding during the sexual intercourse.
The surgery is not difficult for an experienced surgeon, it is better to perform the surgery under general anesthesia.
